Which exam board?
Most people are happy to work towards a grade exam of the ABRSM (Associated Board) which is almost an industry standard. Personally I like competition so I'm happy to work with Trinity or LCM (London College). With my own practical pupils (piano and keyboard) I work almost exclusively with the ABRSM.
ABRSM
ABRSM (Associated Board) has a well organised program of theory exams and support materials (books and past papers etc). If you want to sit one of their grade 6 practical exams you need to pass the grade 5 theory exam. You can cram it all into a 4 or 5 month period before your grade 6 practical but I honestly do not recommend that: it is far better to learn it gradually over the preceeding years in tandem with your practical work.
Trinity College of Music
The Trinity College of Music and LCM (London) are viable alternatives. It is up to you and what your requirements are.
